Quick Contact is a mail form,that allows a visitor from your site to send you a email, Also has built in captcha to protect you from bots/spammers. it is very basic and easy to integrateto your exisiting site.It will send a email to the specified address in the config. It has the Name/email and message fields only since it's ment for a quick message, you can of course edit the script to add more fields if needed.
